Senior Case Manager - care planning (Philadelphia, PA)

Harborlight Ember Community Partners is a nonprofit in Philadelphia supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 181 people based in Philadelphia. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Philadelphia. • We will tell you what to expect at each step of the hiring process.
