Senior Case Manager - care planning (Omaha, NE)

Rivermark Nest Housing manages affordable housing communities across Omaha. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 13 people based in Omaha. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Omaha. • We will tell you what to expect at each step of the hiring process.
