Senior Case Manager - care planning (Oklahoma City, OK)

Ironwood Drift Care Services provides in-home and community support in Oklahoma City, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Oklahoma City. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, with a steady weekday schedule. • On site in Oklahoma City. • Applications are reviewed on a rolling basis.
