Senior Case Manager - care planning (Oklahoma City, OK)

Highbridge Granite Community Partners Group is looking for a Senior Case Manager. The job centres on care planning for grant reporting, and it would suit someone who likes steady, practical work. You would join a small organisation of about 60 people based in Oklahoma City. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Oklahoma City. • Applications are reviewed on a rolling basis.
