Senior Case Manager - care planning (Oklahoma City, OK)

Westbrook Juniper Civic Services works with county and city offices around Oklahoma City, and we need a Senior Case Manager to lead our care planning. Public service means patience, paperwork and pride in the outcome. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for budget submissions. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Oklahoma City. • Applications are reviewed on a rolling basis.
