Senior Case Manager - care planning (New Orleans, LA)

Stonegate Granite Property Management manages apartment communities around New Orleans, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of about 599 people based in New Orleans. What you will do • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with resident events. • Bilingual English and Spanish.
