Senior Case Manager - care planning (Minneapolis, MN)

Cedar Iron Housing Group manages affordable housing communities across Minneapolis. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 177 people based in Minneapolis. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
