Senior Case Manager - care planning (Minneapolis, MN)

Foxglove Meadow Community Partners is a nonprofit in Minneapolis supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, on a regular day shift. • On site in Minneapolis. • We reply to every applicant.
