Senior Case Manager - care planning (Minneapolis, MN)

Westbrook Oak Health Group is a healthcare provider in Minneapolis with clinics and support teams. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
