Senior Case Manager - care planning (Milwaukee, WI)

Highbridge West Care Services supports families and individuals in Milwaukee with personal care and coordination, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Milwaukee. • We will tell you what to expect at each step of the hiring process.
