Senior Case Manager - care planning (Milwaukee, WI)

Clearpath Rook Housing is a nonprofit housing provider in Milwaukee, and we need a Senior Case Manager to lead our care planning. Housing is our mission and residents are our neighbours. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent assistance. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Milwaukee. • Applications are reviewed on a rolling basis.
