Senior Case Manager - care planning (Memphis, TN)

Rivermark Nest Housing manages affordable housing communities across Memphis. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for rent assistance. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Full-time, on a regular day shift. • On site in Memphis. • We reply to every applicant.
