Senior Case Manager - care planning (Memphis, TN)

Bluepeak Vista Community Partners Group is looking for a Senior Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Memphis. • We reply to every applicant.
