Senior Case Manager - care planning (Madison, WI)

Westbrook Keystone Civic Services provides shared administrative services to local governments around Madison. We are hiring a Senior Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for procurement. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Madison, with some flexibility after the first few months. • We reply to every applicant.
