Senior Case Manager - care planning (Louisville, KY)

We are growing our team at Silverline Willow Community Partners and adding a Senior Case Manager whose main responsibility will be care planning. Flexibility and kindness are expected. What you will do • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
