Senior Case Manager - care planning (Louisville, KY)

Fernwood West Care Services supports families and individuals in Louisville with personal care and coordination, and we need a Senior Case Manager to lead our care planning. We train and promote from within. What you will do • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Louisville, with some flexibility after the first few months. • We reply to every applicant.
