Senior Case Manager - care planning (Kansas City, MO)

Silverline Oak Civic Services works with county and city offices around Kansas City, and we need a Senior Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Kansas City. • We reply to every applicant.
