Senior Case Manager - care planning (Kansas City, MO)

Greenridge Clear Care Services Group provides in-home and community support in Kansas City. We are hiring a Senior Case Manager to take on care planning. You would join a small organisation of about 97 people based in Kansas City. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Kansas City. • We will tell you what to expect at each step of the hiring process.
