Senior Case Manager - care planning (Jackson, MS)

Silverline Forge Care Services provides in-home and community support in Jackson, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 36 people based in Jackson. What you will do • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, Monday to Friday. • On site in Jackson, with some flexibility after the first few months. • We reply to every applicant.
