Senior Case Manager - care planning (Jackson, MS)

Silverline Juniper Civic Services Partners is looking for a Senior Case Manager. The job centres on care planning for procurement, and it would suit someone who likes steady, practical work. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for procurement. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with procurement. • Bilingual English and Spanish.
