Senior Case Manager - care planning (Jackson, MS)

Clearpath Orchard Care Services Group is looking for a Senior Case Manager. The job centres on care planning for licensing, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish.
