Senior Case Manager - care planning (Houston, TX)

Stonegate Lantern Property Management Group is a Houston property manager with leasing, maintenance and resident services. We are hiring a Senior Case Manager to take on care planning. Maintenance and leasing staff share information every day. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease renewals. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Houston. • We reply to every applicant.
