Senior Case Manager - care planning (Hartford, CT)

We are growing our team at Redstone Pike Community Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Hartford, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
