Senior Case Manager - care planning (Grand Rapids, MI)

Harborlight Meadow Community Partners is looking for a Senior Case Manager. The job centres on care planning for food distribution, and it would suit someone who likes steady, practical work. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Grand Rapids, with some flexibility after the first few months. • We reply to every applicant.
