Senior Case Manager - care planning (Grand Rapids, MI)

Fernwood Ivory Health Group runs clinics and patient services around Grand Rapids. We are hiring a Senior Case Manager to take on care planning. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Grand Rapids, with some flexibility after the first few months. • We reply to every applicant.
