Senior Case Manager - care planning (Grand Rapids, MI)

We are growing our team at Bluepeak Basin Civic Services Group and adding a Senior Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Grand Rapids. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Full-time, on a regular day shift. • On site in Grand Rapids, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
