Senior Case Manager - care planning (Grand Rapids, MI)

Harborlight Junction Housing provides affordable homes and support services around Grand Rapids, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Grand Rapids. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Grand Rapids. • We reply to every applicant.
