Northwind Union Health is looking for a Senior Case Manager. The job centres on care planning for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Fresno. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Fresno. • Applications are reviewed on a rolling basis.
