Senior Case Manager - care planning (Fort Worth, TX)

Cedar Bright Community Partners is a nonprofit in Fort Worth supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on care planning. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
