Senior Case Manager - care planning (Fort Worth, TX)

Fernwood Lantern Community Partners is a nonprofit in Fort Worth supporting families with food, housing and job help, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Full-time, with a steady weekday schedule. • On site in Fort Worth, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
