Senior Case Manager - care planning (Fort Worth, TX)

Bright Cedar Health is a healthcare provider in Fort Worth with clinics and support teams. We are hiring a Senior Case Manager to take on care planning. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Full-time, on a regular day shift. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
