Senior Case Manager - care planning (Fort Worth, TX)

We are growing our team at Northwind Sparrow Care Services Group and adding a Senior Case Manager whose main responsibility will be care planning. You would join a large organisation of about 349 people based in Fort Worth. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
