Senior Case Manager - care planning (Columbus, OH)

We are growing our team at Highbridge Cross Civic Services and adding a Senior Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for permits. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with permits. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Columbus. • Applications are reviewed on a rolling basis.
