Senior Case Manager - care planning (Columbus, OH)

Clearpath Mill Care Services provides in-home and community support in Columbus, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 44 people based in Columbus. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records.
