Senior Case Manager - care planning (Cleveland, OH)

Clearpath Pike Property Management manages apartment communities around Cleveland. We are hiring a Senior Case Manager to take on care planning. We are flexible about schedules. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance requests. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Cleveland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
