Senior Case Manager - care planning (Cleveland, OH)

We are growing our team at Clearpath Forge Care Services and adding a Senior Case Manager whose main responsibility will be care planning. Teams support each other, especially on hard days. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, with a steady weekday schedule. • On site in Cleveland. • We will tell you what to expect at each step of the hiring process.
