Senior Case Manager - care planning (Cleveland, OH)

Silverline Ashton Care Services provides in-home and community support in Cleveland, and we need a Senior Case Manager to lead our care planning. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Cleveland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
