Senior Case Manager - care planning (Chicago, IL)

Highbridge Mill Care Services supports families and individuals in Chicago with personal care and coordination, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for client records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Chicago, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
