Senior Case Manager - care planning (Chicago, IL)

Westbrook Summit Property Management Group looks after rental homes across the Chicago area. We are hiring a Senior Case Manager to take on care planning. Residents are neighbours, not numbers. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Chicago. • We reply to every applicant.
