Senior Case Manager - care planning (Chicago, IL)

Bluepeak Rowan Civic Services Group supports public agencies in Chicago with budgeting, records and programmes, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 13 people based in Chicago. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for procurement. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Full-time, Monday to Friday. • On site in Chicago, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
