Senior Case Manager - care planning (Charlotte, NC)

Ironwood Oak Health Partners runs clinics and patient services around Charlotte, and we need a Senior Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Charlotte. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
