Senior Case Manager - care planning (Charleston, SC)

Stonegate Quill Health provides primary and specialty care in Charleston, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 149 people based in Charleston. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Charleston. • Applications are reviewed on a rolling basis.
