Senior Case Manager - care planning (Buffalo, NY)

Westbrook Kestrel Housing provides affordable homes and support services around Buffalo, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 174 people based in Buffalo. What you will do • Help with applications and appointments for resident services. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish.
