Senior Case Manager - care planning (Buffalo, NY)

We are growing our team at Lumen Bright Housing and adding a Senior Case Manager whose main responsibility will be care planning. Collaboration with local agencies is constant. What you will do • Help with applications and appointments for maintenance stores. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Buffalo, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
