Senior Case Manager - care planning (Boise, ID)

Lumen West Care Services provides in-home and community support in Boise, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 16 people based in Boise. What you will do • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Boise, with some flexibility after the first few months. • We reply to every applicant.
