Senior Case Manager - care planning (Boise, ID)

Bluepeak Orchard Health provides primary and specialty care in Boise. We are hiring a Senior Case Manager to take on care planning. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boise. • We will tell you what to expect at each step of the hiring process.
