Senior Case Manager - care planning (Birmingham, AL)

Rivermark Mill Care Services Group provides in-home and community support in Birmingham, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 200 people based in Birmingham. What you will do • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, on a regular day shift. • On site in Birmingham. • We reply to every applicant.
