Senior Case Manager - care planning (Birmingham, AL)

We are growing our team at Harborlight Mill Care Services Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Birmingham. • We will tell you what to expect at each step of the hiring process.
