Senior Case Manager - care planning (Baltimore, MD)

Silverline Ashton Civic Services is looking for a Senior Case Manager. The job centres on care planning for constituent requests, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Baltimore. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Baltimore, with some flexibility after the first few months. • We reply to every applicant.
