Senior Case Manager - care planning (Austin, TX)

Highbridge Cedar Civic Services Group is looking for a Senior Case Manager. The job centres on care planning for constituent requests, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Austin, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
